Top Ten Things To Know Cardiac Arrest in the Emergency Department |
NRCPR Investigators found that: 1. There is limited comparative data for cardiac arrest in the emergency department (ED) versus other inpatient areas. 2. Cardiac arrest patients in the emergency department have better survival compared to patients arresting in other hospital locations. 3. This is a call to action for improved resuscitation efforts in hospitalized patients. |

CO2 and CPR |
Nearly three m illion intubations are performed outside the operating room in the United States each year. A review of literature reveals that approximately 1% to 24% of intubation attempts may result in esphageal intubations, which carry a significant risk of morbidity and mortality. |

Commitment to Respond to COMMIT/CCS-2 Trial Results |
As the largest clinical trial ever undertaken in China and the second largest trial ever conducted in this patient population in the world, the preliminary results have raised questions about the use of beta blockers (IV then oral dosing) in the early stages of MI, especially in Killip Class II and III heart failure patients. |

Hyperventilation-Induced Hypotension During Cardiopulmonary Resuscitation |
A clinical observational study revealed that rescuers consistently hyperventilated patients during out-of hospital cardiopulmonary resuscitation (CPR). The objective of this study was to quantify the degree of excessive ventilation in humans and determine if comparable excessive ventilation rates during CPR in animals significantly decrease coronary perfusion pressure and survival. |

Increasing Amiodarone Use in Cardiopulmonary Resuscitation |
To examine practice patterns of amiodarone during in-hospital cardiac arrest, National Registry of Cardiopulmonary Resuscitation (NRCPR) investigators studied the changing pattern of use over time following the inclusion of amiodarone for treatment of ventricular fibrillation / pulseless ventricular tachycardia (VF/pVT) in the American Heart Association Guidelines for Cardiopulmonary Resuscitation and Emergency Cardiac Care in 2000. |

Delayed Time to Defibrillation After In-Hospital Cardiac Arrest |
NRCPR Investigators found that 30.1% of patients with cardiac arrests due to ventricular arrhythmia underwent defibrillation more than 2 minutes after initial recognition of their cardiac arrest. Patients with defibrillation delayed >2 minutes were significantly less likely to survive to hospital discharge; and among survivors, patients with delayed defibrillation were more likely to have major disabilities in neurologic or functional status. |

Vasopressin for In-hospital Pediatric Cardiac Arrest |
NRCPR Investigators found that 1. There is infrequent use of vasopressin with pediatric cardiac arrest. 2. When vasopressin is used it is usually given in combination with epinephrine 3. Lower return of spontaneous circulation (ROSC) is associated with vasopressin use. |
